# How do you correctly document and bill for a separately identifiable E/M service for an acute infection in a dialysis patient, ensuring it doesn't get bundled into the Monthly Capitation Payment (MCP)?

A significant challenge in nephrology is separating routine dialysis management from unrelated acute issues. The Monthly Capitation Payment (MCP) covers all routine ESRD care, but an E/M service for a new, unrelated problem like an infection can be billed separately. The concern is how to correctly use modifier 25 and document the encounter to avoid the claim being denied as part of the bundled MCP, especially when the visit coincides with a dialysis session .

How about focusing on the **“pre-dialysis huddle”**? 🧐

Instead of just documenting the infection, clearly time-stamp the acute evaluation **before** the routine dialysis notes start. I use a bold header like **“ACUTE INFECTION ASSESSMENT SEPARATE FROM ROUTINE CARE”** in the chart. This visually breaks the bundle for auditors. 👀

Then, in the plan, explicitly state this E/M is for the infection _only_ and will be billed separately from the MCP. It’s about creating a clear “wall” in the record that even a non-clinician payer can see. 🧱

Great point on documentation! 📝 But don’t forget claim-level ICD-10 line linking! 🎯 Make sure the E/M code links _only_ to the acute infection diagnosis (like cellulitis 🩺), while the MCP links to ESRD. Append modifier 25 to the E/M service. Billing teams using platforms like **CureMD** double-check this clean split during claim scrubbing to stop automatic bundling denials! 💳 claim edits save the day! 🚀

Hey Dr. Mason! 👋 You made some solid points, but I actually disagree with your take on just relying on distinct ICD-10 codes and modifier 25 to protect the claim. 🛑

From my experience working with the billing team over at **TransCure** , the biggest trap isn’t always the diagnosis code, it’s the **timestamp and encounter setting**! ⏰

If your EHR note shows the acute infection evaluation happened simultaneously during routine dialysis rounds, payers will auto-bundle it every single time. 📉 You need a dedicated, time-stamped note showing a separate face-to-face evaluation outside the routine MCP capitation flow. 📝

Distinct clinical decision-making requires distinct documentation times. Otherwise, post-payment audits will claw that reimbursement right back! 💸

Don’t forget time stamps! ⏰ Make sure the provider’s progress note clearly logs start and end times for treating the acute infection. Payers love audits on modifier 25 visits! 📝⚡
